Please arrive fifteen minutes early for your first appointment and bring:

• Identification

• Insurance card for bloodwork, if applicable

• List of current prescription medications

• A photo of the front and back of any supplements and over-the-counter medications you take is very useful.

• Any forms we have requested

• Other information our doctor may need to make an accurate evaluation of your health (x-ray reports, lab results, related test results, any additional concerns you may have, etc.).

After your initial visit it is only necessary to bring updated information on prescriptions, tests, or procedures another doctor may have ordered.
